The Pharmacist - Clinical Specialist coordinates medication use, related education, and research to provide pharmaceutical care to patients.
About the Job:
Full time with minimal weekend responsibility
Manages drug therapy protocols as a part of an interdisciplinary team.
Reviews, interprets and evaluates medication orders to identify, prevent and resolve potential or actual drug-related problems.
Provides drug information and education to medical staff and patients.
Documents interventions and their outcomes for use in drug therapy quality improvement efforts.
Licensed Pharmacist obtained prior to hire date or job transfer date
Preferred Certification: BCPS or BCCCP
Applicants must possess a Doctor of Pharmacy degree from a college accredited by the American Council on Pharmaceutical Education and must hold or be eligible for pharmacist licensure in Indiana.
Applicants must have completed an ASHP-Accredited PGY1 pharmacy practice residency or equivalent experience and completed an ASHP-Accredited PGY2 cardiology/critical care residency or equivalent experience
Minimum Qualification: Bachelor's Degree in Pharmacy by hire date to position and successful completion of PGY1, and PGY2 in cardiology/critical care (or equivalent experience)
Preferred Qualification: Doctorate in Pharmacy by hire date to position
Minimum Qualification: One year of experience working as pharmacist within a clinical practice or hospital setting.
