Financial Disclosure Report: Not required The VA Palo Alto HCS is building out its Clinical Command Center (C3) aimed at coordinating key operational services to optimize patient flow throughout the health care system. Using a data driven approach to system-wide facility flow, continuous improvements to delivery of efficient and optimal patient care will ensure safe, high-quality, and patient-centered care. The incumbent is a physician who will report directly to the Deputy Chief of Staff for Acute Care Hospital Operations and serve as the Co-Lead of the Clinical Command Center in a dyad partnership with the Nurse Director of the Clinical Command Center. The physician serves as an organizational leader and one of the primary points of accountability in the service working to maximize efficient use of available resources. The physician will maintain high clinical standards to provide guidance on the decision-making processes surrounding patient flow, interdisciplinary coordination, resource use and allocation. This physician will be responsible for the leadership, education, and training of professional staff at VAPAHCS as related to C3 goals with a significant portion of time (up to 0.80 FTE) dedicated to improvement efforts as they relate to patient flow, utilization management, patient safety, and facility flow performance metrics. The remainder of the time will be spent in direct patient care activities. Duties to include: Ensure a positive working relationship with service leaders, frontline physicians, and other staff members to create a culture of open communication and mutual understanding between physicians and facility leadership. Demonstrate a leadership style that is facilitative and collaborative, modeling behavior consistent with the mission, vision, and values of the organization through leading, coordinating projects, innovation, initiating improvement, and developing new processes. Understand the current facility-wide strategies at VAPA HCS to delivering the right care, in the right place, at the right time Develop goals and objectives for the Clinical Command Center based on National, VISN, and facility strategic priorities in alignment with institutional policy and goals. Provide operational oversight and collaborate closely with the Clinical Command Center team to streamline inflows and outflows for patient movements within the health care system ensuring effective operations of the Center Build coalitions to identify and execute on major improvement opportunities using a data driven approach and patient-centered principles that will allow staff to examine and redesign health care delivery processes with the goal of eliminating waste and providing right care at the right time for the right patient Lead Process Improvement efforts as they relate to the Clinical Command Center, patient flow, and performance metrics. Standardize services, processes, resources, and practice to improve efficiency and ensure hospital capacity can meet demands for hospital services. Monitor flow processes including but not limited to: daily acute care patient flow from all areas to acute inpatient beds; timely transfers from within the facility: Mental Health, Rehab, ECS, and transfers to and from the community, outside hospitals, or other VA facilities. Serve as Co-Chair for the Patient Flow Committee in partnership with the Nurse Director of the Clinical Command Center Serve as a Physician Utilization Management Advisor (PUMA) and collaborate with facility Utilization Management staff on referred cases that do not meet medical criteria, including evaluation and documentation of secondary reviews. Participate on interdisciplinary committees related to the Clinical Command Center, patient flow, and/or facility performance metrics as assigned. Serve as supervisor to assigned Command Center staff and perform all necessary administrative performance appraisals. Provide specialized diagnosis and treatment of acute inpatient diseases to veterans at VAPAHCS. Serve as primary attending for patients in Med/Surg and the Intermediate ICU wards Diagnose, treat, and manage patients with acute inpatient diseases according to accepted standards of care. Supervise, teach, and mentor medical residents and medical students; participate and promote educational activities within the section of inpatient medicine. Participate in Process Improvement/Quality Assurance projects and other administrative duties as assigned. Participate in the various educational programs of the Section of Inpatient Medicine. Participate in monitoring the quality, safety and appropriateness of patient care. Demonstrate ethical conduct and serve as a positive role model to others at all levels of the organization. Participate in quality assurance programs and chart reviews. Attend Medical Service staff meetings and Medical Staff meetings. Complete medical records in accordance with Bylaws and Rules and Regulations of the Medical Staff.
