This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more This position is for Primary Care Nurse Practitioner which is aligned under Primary Care Service. The provider will work in the Detroit VA Medical Center and based on the needs of the service the Provider could also work in the Veteran's Community Resource and Referral Center (VCRRC) and the Pontiac CBOC. The Provider will be providing primary care for Veterans. The Nurse Practitioner is a licensed independent practitioner that assists with the total care of the patient's medical needs by performing history and physical exams, ordering appropriate lab tests, and evaluating those tests, to ensure quality care of the patient. This will also include providing preventive medicine services, managing chronic disease problems and providing outpatient follow-up as needed. The Nurse Practitioner conducts a comprehensive assessment of patients' medical, physical and biopsychosocial status and manages/evaluates outcomes of care, including: Obtains a relevant health and medical history Performs physical examination Conducts preventive screening procedures based on age and history Updates evaluation of health status to determine need for further health care Formulates appropriate diagnostic procedures and laboratory tests. Identifies and prescribes appropriate pharmacological and non-pharmacological agents. Identifies needs of the individual and family as a result of the evaluation of collected data. Provides relevant patient/family education. Makes appropriate referrals to both health professional and community agencies. Consults with collaborating health care professional when doubt exists as to the diagnosis, when expected. improvement is not soon apparent and when specialized therapeutic or diagnostic techniques are indicated. Determines the effectiveness of the plan of care through documentation of client care outcomes. Reassesses and modifies the plan as necessary to achieve medical and health goals. Collaborate with primary care physician and specially providers on a routine basis. Plans, develops, implements and evaluates patient education tools and clinic forms. Develops and evaluates professional educational programs in areas of expertise. Presents educational programs for patients in areas of expertise. Participates in patient record review at least annually with collaborating physician or designee. Outcome of record review will be discussed with collaborating physician/designee and implemented through PI process.
