Primary Responsibilities:
- Advanced Practice Provider will conduct home visits
- Examines, diagnoses and treats patients for acute injuries, infections, and illnesses
- Counsels and educates patients and families about acute and chronic conditions or concerns
- Documents items such as: appropriate chief complaint, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment and plan
- Formulates diagnostic and treatment plans
- Prescribes and administers medications, therapies, and procedures
- Orders lab and imaging tests to determine and manage an immediate treatment plan and provides advice on follow up
- Responsible for the coordination of care with specialists and appropriate ancillary services
- Completes all documentation and paperwork in a timely manner
- Maintains quality of care standards as defined by the practice

Required Qualifications:
- Advanced Nurse Practitioner Certification in Primary Care, Geriatric, Adult or Physician Assistant certification
- Active unrestricted APRN or PA-C license in Nevada, or ability to obtain
- 2+ years of home health or clinical practice experience as an NP/PA
- Ability to communicate with patients, primary care team members and management staff
- Ability to make care decisions based on best medical practices without direct supervision or guidance by a physician
- Basic technical proficiency in the use of computer programs for medical records or internet search for clinical practice
- Advanced assessment and treatment plan implementation skills
- Ability to complete clinical care lifting in a provider office environment
- Current and unrestricted DEA certificate required or ability to obtain
- Effective communication skills
- Effective organization skills and ability to multi-task
- Takes initiative, creative, has problem solving ability, is adaptable, and flexible
- Ability to work without direct supervision and practice autonomously
Preferred Qualifications:
- Geriatric care experience
- Knowledge of mental health issues
- Familiarity with Medicare and ICD-10 coding
- Ability to draw blood in the home for lab procedures
- Strong acute care experience in a primary care clinic, urgent care, or retail clinic
- Ability to formulate a differential diagnosis
- Proficient in assessment and development of treatment plans
- Ability to conceptualize the needs for the team or department and actualize plans for implementation
- Ability to develop plans of action and gain team consensus towards common goals
- Managed Care/IPA/Health Plan experience
- Bilingual in Spanish
- Must be able to effectively communicate with elderly and chronically ill patients and families
- Understanding of Medicare, Medicaid and Health Plan benefit structures beneficial
- Comfortable working independently in a home-visit setting and clinic setting

Nevada Residents Only: The salary range for Nevada residents is $87,900 to $156,900 per year.
